# Fackrell & McLean Law QDRO Center

## QDRO Center Instructions

**1. Fill Out the Form**:

- Navigate to the form on this page.
- Both parties will need to fill out a form with their information.

**2. Attach Documents**:

- Only one party needs to attach the required documents.
- Ensure all necessary documents are included.

**3. Alternative Submission Option**:

- One party can submit the form twice, each time with the information of one party. The documents only need to be submitted once.

**4. Sign the Retainer**:

- Follow the link provided on this page to sign the retainer.

**5. Make the Payment**:

- Follow the link provided on this page to complete the payment.

## **Payment**

Click the button below to pay the retainer amount. Each QDRO is $600 total. If each party is paying 1/2 of the cost, it is $300 per QDRO per Party.
